This practice is taking part in a new NHS service that helps your family doctor (GP) to spot whether you need more help to manage your health. The service is called "risk profiling".

Using information from your health records, a secure NHS computer system will look at any recent treatments you have had in hospital or at this surgery, and any existing health conditions that you have, and alert your doctor to the likelihood of a possible future hospital admission. The clinical team at the surgery will use the information to help you get early care and treatment where it is needed. The information will be seen only by qualified health workers involved in your care. NHS security systems will protect your health information and patient confidentiality at all times.

Electronic Patient Record & the Sharing of Information

Longroyde Surgery’s Clinical System - TPP SystmOne, has a new patient record sharing model which will be implemented across all SystmOne modules during the course of September 2013.

Within Calderdale CCG all patient records within GP Practices and Community Services who use SystmOne will be upgraded to the enhanced data sharing model (eDSM). This new sharing model enables patients to make decisions about sharing their record at their practice and also each community service they are under the care of.   Patients will be able to choose whether to “share out”, i.e. make their record available to be viewed wherever they have also consented to “share in”, i.e. consented to their record being viewed.

This new sharing model is being implemented to bring SystmOne record sharing in line with the National Care Record Guarantee, by enabling patient’s to make choices regarding the sharing of their records.

Please note: If you have previously opted out of sharing your information via the Summary Care Record, you will still need to complete this form with your choices about sharing your health record within SystmOne.
